Obituary for Theresa A. Hendricks
Dunlap-Theresa A. (Prohaska) Hendricks, age 50, of Dunlap, passed away 9:00 AM, Friday, December 2, 2016 at OSF Richard L. Owens Hospice Home in Peoria, IL.

Born May 19, 1966 to Kenneth Prohaska and Vicky L. (Sylvester) Prohaska, she married Michael J. Hendricks on October 10, 1992. He survives, along with two sons; Zachary Hendricks of Bloomington and Johnathon Hendricks of Dunlap, one daughter; Elizabeth Hendricks of Dunlap, one sister; Shelly Prohaska of Peoria, one brother; Richard (Denise) Prohaska of Carrollton, Texas, one niece; Amanda Prohaska of Carrollton, Texas, one nephew Tyler Prohaska of Nashville, TN, and grandmother, Edna (Gebhart) Sylvester of Peoria.

She was preceded in death by her mother and an infant sister.

Theresa was a wonderful wife, mother, sister, aunt, and friend.  A 1984 graduate of Richwoods High School, she worked for over 20 years at Walmart/Sams Club. She was devoted to being the best mother possible to her children, always supporting and volunteering at their practices, events, performances, and activities.  She was proud that her sons are Eagle Scouts from Troop 88.  She enjoyed leading her daughters Girl Scout troop.  She really enjoyed making homemade greeting cards and was great at candy making and baking – even her mistakes were rarely thrown out.

We want to thank all the family, friends, neighbors, and OSF Mission Partners that have supported us all, as well as all of the caregivers that fought alongside her during her battle with gliosarcoma brain cancer including Dr. Marc Squillante of OSF Saint Francis Medical Center, Dr.’s Mackenzie McGee, Andrew Tsung, and APN Sarah Beam - all of the OSF Illinois Neurological Institute (INI), and Dr. Francois Geoffrey of OSF INI at the Illinois Cancer Center.  Special notes of appreciation to Matt Davis Custom Homes for helping make the house accessible, and the Dunlap Fire Department and all of the staff of OSF Home Hospice and the OSF Richard L. Owens Hospice Home with the guidance of Dr. Patricia Deters, who compassionately helped Theresa maintain her dignity and comfort as the fight concluded.

Memorials in Theresa’s memory may be made the OSF Richard L. Owens Hospice Home.

Visitation will be 4:00 – 6:00 PM, Tuesday, December 6, 2016 at Wright & Salmon Mortuary. Funeral Mass is 10:00 AM Wednesday, December 7, 2016 at St. Jude Catholic Church in Peoria, IL, with a half hour prior visitation. Father Patrick Henehan will officiate. Burial will follow in St. Clement’s Cemetery in Dunlap, IL.
